ABOUT US

Gateway Health is a community health service providing health and welfare services to individuals, families and communities across north east Victoria and parts of southern New south Wales. We are a not-for-profit Company limited by guarantee and a registered Community Health Service under the Health Services Act 1988 (Vic).

The organisation employs over 380 staff providing a range of primary health and community services across the Ovens Murray region in North East Victoria and parts of Southern New South Wales. Gateway Health serves all people, and is committed to improving individual and population health outcomes. We do this by providing health care and support to individuals and communities in times of need, and by actively working with our partners to address the social and environmental determinants of health.

ABOUT THE ROLE

This position plays a pivotal role in the delivery of the Aged Care Assessment Program (ACAP). The focus of the role is to conduct assessments to work out whether an older person is eligible for government-subsidised aged care. Assessors visit the older person’s home or hospital to:

  • Understand the person’s care needs, goals and preferences.
  • Decide on the types of services they may be eligible for
  • Create a goal-oriented support plan based on their care needs and preferences.

Assessments are completed using the Integrated Assessment Tool (IAT), and all staff must successfully complete training in the use of this tool, as well as completing all assessor training requirements of the Department of Health and Aged Care. This will include delegate training once assessors meet eligibility requirements for this role.

All assessors use the My Aged Care assessor portal to: manage referrals, record comprehensive assessments, develop support plans, recommend services, record approvals for care, refer a person for services, and review and update central client records.

Assessors work both independently, and in partnership and collaboration with other members of a high functioning multidisciplinary team.

ABOUT YOU

We’re looking for candidates with:

  • Qualifications and experience in any of the following disciplines: Nursing (RN only), Occupational Therapy, Physiotherapy or Social Work.
  • A passion for understanding the individual needs of older people, respecting their choices, preferences and diagnosed conditions, and understanding the journeys of their life.
  • Transferable skills in aged care, quality, clinical, and interviewing assessment.
  • Experience in gathering information, then corroborating the evidence from alternative sources and presenting this in report format.
  • Report writing skills, able to write focussed, concise, and informative reports.
  • Knowledge and experience in restorative care and rehabilitation and/or wellness and reablement approaches.
  • The ability to work autonomously and as part of a dynamic, professional and supportive team.
