Nurse Practitioner

The Nurse Practitioner will be responsible for working collaboratively with clients, practice staff and other health services to coordinate multidisciplinary care and services to clients of Waminda, with significant chronic health and other medical issues within the guidelines of the program. This role will support the Clinical Governance of the clinical services offered and will be required to have a good skill mix and be able to work within general practice, showing leadership and collaboration with all of the team. The Nurse Practitioner will provide competent patient centered care that addresses the health needs and promotes wellness. Such care is to be based on the latest evidence based guidelines and meet legislative requirements. The Nurse Practitioner will:

  • Develop and support the implementation of care plans with support from other Health Professionals within the service including General Practitioners, Nurses, Midwives and Aboriginal Health Practitioners/Workers;
  • Work with clients that present to the service to ensure their health needs are addressed, by providing skilled health assessments, diagnosis and treatment services;
  • Be responsible for ordering diagnostic tests as required, reviewing and informing clients of their results and providing appropriate care as per the polices and procedures of the service;
  • Refer clients appropriately to other health services and providers, including specialists, if their needs exceed the range of care able to be provided by the service;
  • Conduct health assessments including Team Care Arrangements and Management Plans and link clients with culturally appropriate primary health care, allied health and specialist services needed to manage chronic conditions;
  • Support and assist clients to develop an understanding of their chronic condition or medical issue and its day to day management;
  • Consult and collaborate with colleagues to provide optimal care for clients;
  • Work closely with clients, carers and primary health care teams regarding coordination and continuity of care;
