ABOUT THE ROLE

We are currently looking for motivated and passionate Assessment Officers to join the supportive and friendly Aged Care and Disability Support team. As a Regional Assessment Officer, supporting clients in the Hume region, you will conduct face-to-face and telehealth home support assessments with older Australians to enable access to the care they need to remain living independently in their chosen community. You will use a standardised national assessment process that focuses on wellness.

Working with each client, you will establish goals specific to the client and develop appropriate strategies and actions documented within a Support Plan, including referral to age care and non-aged care services. The Assessment Officer completes the Integrated Assessment Tool (IAT) and matches and refers clients to appropriate service(s).

A typical week as an Assessment Officer includes:

  • Travelling to the client’s home to conduct the My Aged Care assessment as part of the Regional Assessment Services (RAS) program.
  • Undertaking client-centred support planning using a wellness and reablement approach. 
  • Liaising with referral sources and community support organisations to assist our clients to obtain the home supports they require.
  • Communicating in person, over the phone and in writing with clients, families, service providers, your team and other program contacts during the assessment process.
  • Developing and reviewing support plans, generating assessment summaries and case notes that document the client’s journey.
  • Ensuring accurate and timely completion of assessments and documentation, utilising the My Aged Care online portal. 
  • Working as part of a skilled team to expand your knowledge, networks and career.

Important to the role is the ability to quickly build rapport, to communicate effectively with a range of people and to identify the support required, so that the person can continue to live a full and independent life.

ABOUT YOU

To be successful in this role you will have the client’s needs at the forefront of everything you do. You will have a great friendly attitude, have excellent organisational skills, be willing to learn, be able to work autonomously and most importantly listen and understand what the customer wants and needs.

Ideally you will have:

  • Completed My Aged Care, Home Support Assessor Training (SOA2) and IAT training, or be able to obtain this within the first three months of employment.
  • Experience in a similar role, preferably aged care and/or community services background. 
  • Ability to work independently, and as part of a team.
  • High level of computer literacy skills
  • Ability to follow guidelines relevant to the position as well as demonstrated problem-solving capacity.

Dependent on your qualification:

Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi Enterprise Agreement 202, Classifications Level 4

or

Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2020-2024, Classification Enrolled Nurse Level 2

or

Victorian Stand-Alone Community Health Services (Health and Allied Services, Managers & Administrative Officers) Multiple Enterprise Agreement 2022-2026, Allied Health Assistant Classification Grade 3
