ABOUT THE ROLE

We are looking for a Coordinator for our home and Community Care program in Benalla. As an integral member of our team, you will be encouraged to grow, develop and feel empowered to make the everyday easier for our staff to work their passion in providing client centred care.

The Coordinator will report to the Program Manager Aged Care and Disability and be responsible for the implementation and coordination of the HACC PYP program. The Care Coordinator works collaboratively with HACC-PYP participants, their carers, support workers and other agencies to ensure that a safe, equitable and high-quality service is provided which assists participants to meet their needs and achieve their goals.

You’ll have the opportunity to lead and inspire your team providing supervision, guidance, and mentorship, through your effective decision making and extensive experience. This includes:

  • Liaising with GPs and other health and welfare agencies to provide information about the HACC-PYP program and encourage referrals.
  • Undertaking intake, assessment, planning, care coordination and review processes with participants.
  • Liaising with the rostering officer to ensure that supports and services are provided in a timely, effective and efficient manner.
  • Working effectively with the Community Inclusion Team Leader to ensure all data collection, reporting and compliance activities are performed accurately and on time.

ABOUT YOU

You’ll be an advocate for person-centred care, leading by example through exceptional communication and customer service. You’ll present with:

  • Qualifications in a relevant field, such as: Social Work, Welfare or Community Services, Service Coordination, Case Management, Allied Health or Disability-specific qualifications at Advanced Diploma Level. 
  • Experience coordinating care provision within a small to medium sized team.
  • Confidence to manage conflict, pressures, and competing priorities.
  • Current Working with Children’s Check and Police Check.
  • A current Victorian Driver’s license and an ability to travel.
  • The ability to work autonomously.
