About the roleThe Indigenous Outreach Worker will provide the core of service with OIWs conducting scheduled check ins utilising the covid wellbeing check clinical item and scripts on clinical indicators of issues of concern along with social well-being concerns.•You’ll IMPACT the community, helping close the gap in Indigenous healthcare and wellbeing, one helping hand at a time. •You’ll be PROUD, both work you do and who you work for. •You’ll work with a TEAM, alongside people who are down to earth and truly dedicated to what we do. •You’ll EXPERIENCE and learn something new every day through the variety of your role.•You’ll embrace the OPPORTUNITY to progress your career – follow your path at Danila Dilba. Responsibilities include: •Maintain regular contact with clients within the scope of practice that have been identified for the Covid care.•Escalate clinical concerns immediately to the Covid Care Coordinator•Refer to SEWB clinicians if there are mental health/SEWB concerns•Review client records with the RN/AHP.•Perform daily phone calls to moderate risk COVID clients and escalate care accordingly Are you the right person? The successful candidate will have: 1.Demonstrated ability to interact and communicate with Aboriginal people from diverse backgrounds, with the sensitivity to Aboriginal culture, history, and current social impacts on them.2.Demonstrated ability to work within and maintain strict confidentiality guidelines and policies.3.Demonstrated communication and interpersonal skills with the ability to liaise with people from a wide range of cultural and social backgrounds to both obtain and convey information relevant to their care.4.The ability to interact with a range of health professionals including medical practitioners and allied health professionals.We want you to be proud of both the work you do and of who you work for!
